Assessment Transfer Form

This form is required to initiate the transfer of assessment records to a new Provider.
Allow 5-7 business days to complete assessment transfers.

Instructions

  1. The Provider who is receiving the records needs to fill out this form in its entirety.
  2. Along with this form, you must include an email from the client to you, confirming their intention to move the assessment records and proof that the former Account Administrator has been notified. (See section 5)

Tip: To find the EPIC Account ID, click on the link to login. From the top menu, select Personal Options > View/Edit Account Information, and select the EPIC Settings tab. Once there, scroll down to find the Account ID under Company Information.


Step 1: Transfer Information

* Administrator/Company name can be found on the assessment cover page.

DESTINATION: Where should the accounts/assessments be moved?

If this request includes Catalyst records: 


  • The Catalyst™ Destination Account MUST have the Organization in their EPIC Account in order for the records to be moved.
  • Catalyst records can only be transferred to other EPIC Accounts that have their Organization.
  • Catalyst records should be placed in separate folders apart from traditional (e.g. EPIC) records.
  • Folders that contain multiple Catalyst Organizations cannot be moved. They need to be in separate folders.

Step 2: Assessment Information

RECORD(S): Which assessments should be transferred?
b) Unassign any pending access codes and reclaim the associated credits. Records in pending status cannot be moved.

NOTE: Wiley is unable to process any transfer requests for individuals who reside in a country/state regulated by privacy laws without confirmation of consent.
PARTNER AUTHORIZATION:
Client's request to move Records: The client whose records are being moved must email the Account Owner where the assessments currently reside, notifying them that they are moving their assessments to another Account Owner.
